MANAGEMENT OF ALLERGIC STOMATITIS CAUSED BY CHICKEN AND EGG Ganesha, Raziv

Abstract

Introduction: Aller gic stomatitis is a hypersensitivity reaction caused by an allergen such as medicines, foods, dental materials. Chicken and egg are one of the common food we usually consumed. Chicken contains low amount of protein and fat, while egg contains nutrient and easy to process. Although allergic reactions in poultry products are commonly found in eggs and feathers, chicken is often as a cause of the allergic reaction. Case: 28-year-old male patient complained an ulcer in his mouth. It shows past 3 days on his upper lips then it shows on his left cheek. Case Treatment: Diagnosis made by anamnesis, clinical examination and supporting examination such as complete blood count, total IgE, and Skin Prick Test. The patient was prescribed by topical anti-inflammatory 2 times daily and after Skin Prick Test was done the patient was given methylprednisolone 3times daily for 7 days, patient was treated in 3 visits. Discussion: A Food allergic usually involved type 1 and 4 hypersensitivity reaction, or both. Skin Prick Test is one of the skin test that commonly used by clinicians to prove the presence of specific IgE bound to skin mastocyte cells. Treatment for this case beside of medication was advised to avoid food that cause the allergic reaction. Conclusion: Management of this case can be successful if clinician understand the clinical manifestation of hypersensitivity reaction to food and can made the right diagnosis to provide proper treatment. Furthermore requires patient cooperation to follow the prescribed care instruction.

Abstract

Introduction : Herpes simplex virus (HSV) is one variant of the herpes virus that infects humans, has two main types of HSV-1 that manifests more found in the mucosa of the mouth, pharynx, and skin (upper waist), and the type HSV-2 manifests in the genital area (lower waist). Case: 16-year-old female patient were referred from dermatologis RSAL Dr Ramelan Surabaya with complaints of many ulcers, whitish area of the tongue, palatum, vestibulum and genitals. Case Treatment: patients treated with systemic drugs of acyclovir, fluconazole, folic acid, vitamin B, vitamin C, methyl prednisolone, aloe vera extract topical. Patients were tested for blood, HSV-1 & HSV-2 (+), HIV (-), ANA (-). Discussion: Herpes simplex infection is a disease caused by the herpes simplex virus (HSV) which is a DNA virus (Deoxyribonucleic acid). In humans, the herpes virus can be latent and reactivate Reactivation can occur because of a trigger in the form of decreased immunity due to weather changes, fever, sun exposure, emotional stress, trauma, pregnancy, hormonal, systemic diseases, allergies, and immunosuppression. In this case the cause is thought to be due to hormonal. The diagnosis of primary HSV infection and recurrent infection is usually obtained from clinical features and laboratory tests are used establish the diagnosis. Conclucion: Recurrence of HSV infection can be reduced if avoid trigger factors. HSV infection management aimed at the control of pain, supportive treatment and definitive treatment. In this case, multidisciplinary collaboration is required in the treatment because it involves two sites, oral and genital. Keyword : HSV, Recurrent Herpes Oral, Oral Candidiasis

Abstract

Introduction: Primary Herpetic Gingivostomatitis (PHGS) is a primary infection to Herpes Simplex Virus type 1 (HSV-1). PHGS often occurs in pre-school-aged children, but also occur in school-age children and teenager. This case report purpose to discuss management of PHGS in teenager. Case: Patient a 12-year-old with chief complaints are many ulcers in mouth. Ulcer appears after the patient has a fever for 2 days. The patient has never experienced like this before. On Intra Oral examination found multiple ulcers, varying in size, pain in the upper labial, lower labial, lower gingival and lower mucobucalfold. Case Management: patient was given a complete blood count and IgG antigen HSV-1, with reactive results. Patients received treatment in the form of aloe vera extract gel, Immunomodulator syrup. ulcers healed after 12 days of treatment. Conclucion: PHGS is a case that often occurs in pre-school age but also occur in school-age children and teenager. Anamnesis, objective examination and supporting examinations is needed to diagnosis to choose proper therapy.

Abstract

Introduction: Herpes Labialis is the most common form of orofacial herpes, beginning with a feeling of vesicle formation at the vermilion margin of the lips and the surrounding area within 24 hours and then rupture, superficial erosion occurs which is then covered with crusts. Herpes labialis occurs in 50-75% of individuals exposed to HSV-1 infection in the oral cavity and often has a recurrence followed by prodromal symptoms. Reactivation can occur due to triggers in the form of decreased immune conditions due to weather changes, fever, sun exposure, emotional stress, trauma, menstruation, systemic diseases, allergies, and immunosuppression. Case: 22 years old female with a complaint of a wound on the angular lip, initially the patient had a fever for 2 days then a fluid-filled blister appeared which then rupture. Case management: The diagnosis is made using anamnesis, clinical and supporting examination. CBC and Antybody Titer HSV-1 show value under normal for eosinophil and reactive for IgG HSV-1 established. Patient take medicine topical antiinflammation 3 time a day and multivitamins 1 time a day. Discussion: HSV-1 infektion is latent and can reaktivation. Under Stress Condition can caused a reduced circulating population of immune cells such as IL-6, TNFα, Lymphocites B, NK cell, dan Monocytes which HSV-1 Reactivation. Conclusion: Management in case can be successful if the clinician understand HSV-1 Infection Manifestation and added Suporting Examination. In Addition, cooperation from the patien is needed to manage stress so that the disease no more recurrence
